Abstract:Objective: To investigate the relationship between myocardial bridge (MB) in the mid段 of the left anterior descending artery (LAD) and plaque distribution in patients with coronary atherosclerosis, as well as its impact on plaque stability. Methods: A total of 389 patients with coronary atherosclerosis who were treated at Yulin University Medical College from November 2022 to November 2023 were selected. According to the depth of myocardial coverage of the coronary artery, they were divided into three groups: incomplete MB group (coronary artery not completely covered by myocardium, 94 cases), complete MB group (coronary artery completely covered by myocardium, 108 cases), and non-MB group (187 cases). Multivariate logistic regression analysis was used to analyze the relationship between MB in the mid段 of LAD and plaque distribution and stability. Results: There were statistically significant differences in age, hypertension, hyperlipidemia, smoking history, alcohol consumption history, diabetes, cholesterol, obesity, strenuous exercise, MB thickness, systolic and diastolic stenosis degree of MB among the three groups (P < 0.05). There were statistically significant differences in plaque distribution in the proximal, middle, and distal segments of MB between the complete MB group and the incomplete MB group (P < 0.05), but no significant differences between the groups (P > 0.05). There were statistically significant differences in plaque distribution in the proximal, middle, and distal segments of LAD within the three groups (P < 0.05). There was no statistically significant difference in plaque size and degree of stenosis among the three groups (P > 0.05). Age, smoking history, hypertension, hyperlipidemia, diabetes, MB thickness, systolic stenosis degree of MB, and diastolic stenosis degree of MB were risk factors for atherosclerotic plaques in the mid段 of LAD. Conclusion: Plaques were more commonly distributed in the proximal segment of MB than in the MB segment or distal segment. The incidence of plaques in the mid段 of LAD was higher, while that in the distal segment was lower. This suggests that MB may play an important role in coronary atherosclerosis.
