Meta-analysis of the Effects of Early Versus Delayed Coronary Angiography in Patients with Out-of-hospital Cardiac Arrest without ST-Elevation

Abstract:Objective: To evaluate the effect of early versus delayed coronary angiography (CAG) in patients with out-of-hospital cardiac arrest (OHCA) without ST-segment elevation. Methods: Randomized controlled trials (RCTs) comparing the effects of early and delayed CAG in OHCA patients without ST-segment elevation were searched in databases including EMbase, Web of Science, the Cochrane Library, PubMed, VIP, WanFang, and CNKI, from the establishment of the database to June 12, 2023. Meta-analysis was performed using RevMan 5.4.1 software. Results: A total of 8 studies were included. The results of the Meta-analysis showed that compared with delayed CAG, there were no statistically significant differences in 30-day all-cause mortality [OR=1.11, 95%CI (0.92, 1.34), P=0.29], all-cause mortality during follow-up [OR=1.01, 95%CI (0.85, 1.20), P=0.89], favorable neurological outcomes during follow-up [OR=0.88, 95%CI (0.71, 1.08), P=0.22], incidence of acute renal failure [OR=1.25, 95%CI (0.85, 1.82), P=0.26], and major bleeding incidence [OR=0.93, 95%CI (0.54, 1.62), P=0.81]. Conclusion: Current evidence suggests that early CAG is not significantly associated with survival rate, improvement in neurological outcomes, incidence of acute renal failure, or major bleeding in patients with OHCA without ST-segment elevation.
