The Predictive Value of Platelet Parameters for Microvascular Occlusion after Reperfusion Injury in Patients with Acute ST-segment Elevation Myocardial Infarction

Abstract:Objective: To observe the changes in platelet parameters after reperfusion injury (MIRI) in patients with acute ST-segment elevation myocardial infarction (STEMI), and to evaluate the predictive value of platelet parameters for microvascular obstruction (MVO). Methods: A total of 172 patients with STEMI admitted to the 83rd Group Army Hospital of the Chinese People's Liberation Army from March 2021 to August 2023 were selected as the study subjects. According to whether reperfusion injury (MIRI) occurred, the patients were divided into the MIRI group (67 cases) and the non-MIRI group (105 cases). Based on cardiac magnetic resonance imaging results, the 67 MIRI patients were further divided into the non-MVO group (41 cases) and the MVO group (26 cases). Clinical data and platelet parameters of the two groups were compared. Multivariate logistic regression analysis was used to identify the influencing factors of MVO after MIRI. The receiver operating characteristic (ROC) curve was used to assess the predictive value of platelet parameters for MVO. Results: There were statistically significant differences between the non-MIRI group and the MIRI group in cTnI, LVEF, incidence of anterior wall MI, PDW, PLT, MPV, and PCT (P < 0.05). Between the non-MVO group and the MVO group, there were statistically significant differences in the proportion of diabetes, cTnI, LVEF, incidence of anterior wall MI, D-dimer, hs-CRP, LDL-C, PLT, MPV, PDW, and PCT (P < 0.05). Multivariate logistic regression analysis showed that diabetes, anterior wall MI, PLT, MPV, PDW, PCT, cTnI, and LVEF were independent risk factors for MVO after MIRI in STEMI patients (P < 0.05). The area under the ROC curve (AUC) of the combined diagnosis using four indicators (PLT, MPV, PDW, PCT) was higher than that of each individual indicator. Conclusion: Platelet parameters are independent risk factors for MVO after MIRI in STEMI patients and can be used to predict the occurrence of MVO after MIRI in STEMI patients.
