The Predictive Value of Serum FT3,TM,and PAF Levels for Major Adverse Cardiac and Cerebrovascular Events in Maintenance Hemodialysis Patients

Abstract:Objective: To explore the relationship between free triiodothyronine (FT3), thrombomodulin (TM), platelet-activating factor (PAF), and major adverse cardiovascular and cerebrovascular events (MACCE) in patients on maintenance hemodialysis (MHD). Methods: A retrospective study was conducted on 80 MHD patients admitted to Cangzhou People's Hospital from January 2018 to January 2020. Patients were divided into non-MACCE group (44 cases) and MACCE group (36 cases) based on whether they experienced MACCE. The levels of serum FT3, TM, and PAF, as well as clinical data, were compared between groups. Correlation analysis was performed among the levels of FT3, TM, and PAF. Logistic regression analysis was used to identify factors influencing MACCE in MHD patients. Receiver operating characteristic (ROC) curves were used to evaluate the predictive efficacy of serum FT3, TM, and PAF levels for MACCE in MHD patients. Results: Compared with non-MACCE patients, MACCE group showed significantly higher levels of TM, PAF, and low-density lipoprotein cholesterol (P < 0.05), longer dialysis time (P < 0.05), and lower FT3 levels (P < 0.05). Correlation analysis showed that serum FT3 was negatively correlated with TM and PAF levels (r = -0.546, P = 0.003; r = -0.682, P < 0.001), while TM was positively correlated with PAF levels (r = 0.647, P = 0.001). Logistic regression analysis showed that FT3, TM, PAF, low-density lipoprotein cholesterol, and dialysis time were risk factors for MACCE in MHD patients (P < 0.01). The area under the ROC curve (AUC) for combined detection of serum FT3, TM, and PAF in predicting MACCE was 0.984, with a sensitivity of 97.99% and specificity of 88.84%, which was better than individual predictions (P < 0.05 or P < 0.01). Conclusion: Serum levels of FT3, TM, and PAF are closely related to MACCE in MHD patients, and their combined detection has good reference value for predicting MACCE in these patients.
