Abstract:Objective: To explore the relationship between serum levels of oncostatin M (OSM), apolipoprotein A5 (ApoA5), and 3-hydroxybutyric acid (3-HIB) in patients with chronic heart failure (CHF) and ventricular remodeling and prognosis. Methods: A total of 104 CHF patients admitted to Nanchong Central Hospital from July 2019 to September 2021 were selected as the observation group, and 113 healthy individuals undergoing physical examination during the same period were selected as the control group. Enzyme-linked immunosorbent assay (ELISA) was used to detect serum OSM and ApoA5 levels, while magnetic resonance spectroscopy was used to detect serum 3-HIB levels. Ventricular remodeling-related indicators were measured in the patients, and according to the prognosis, the patients were divided into the good prognosis group (72 cases) and poor prognosis group (32 cases). Multivariate logistic regression analysis was used to analyze the influencing factors of poor prognosis in CHF patients, and receiver operating characteristic (ROC) curves were used to evaluate the predictive value of serum OSM, ApoA5, and 3-HIB for poor prognosis in CHF patients. Results: Compared with the control group, the serum OSM and 3-HIB levels were significantly increased (P < 0.001), while ApoA5 levels were decreased (P < 0.001) in the observation group. Serum OSM and 3-HIB levels were positively correlated with NT-proBNP, left ventricular mass index (LVMI), right ventricular diameter (RVD), and left ventricular end-diastolic diameter (LVEDD) (P < 0.001), and negatively correlated with left ventricular shortening fraction (LVFS) and left ventricular ejection fraction (LVEF) (P < 0.001). Serum ApoA5 was negatively correlated with NT-proBNP, LVMI, RVD, and LVEDD (P < 0.001), and positively correlated with LVFS and LVEF (P < 0.001). Compared with the good prognosis group, the poor prognosis group showed higher levels of NT-proBNP, LVMI, RVD, LVEDD, serum OSM, and 3-HIB (P < 0.05), and lower levels of LVFS, LVEF, and serum ApoA5 (P < 0.05). Multivariate logistic regression analysis showed that OSM and 3-HIB were risk factors for poor prognosis in CHF patients (P < 0.05), while ApoA5 was a protective factor (P < 0.05). The areas under the ROC curve (AUC) for predicting poor prognosis by OSM, ApoA5, 3-HIB, and their combination were 0.860, 0.856, 0.918, and 0.983, respectively, with the best diagnostic efficiency when combined. Conclusion: Serum OSM, ApoA5, and 3-HIB levels in CHF patients are related to ventricular remodeling, and the combination of the three has certain predictive value for patient prognosis.
