The Prognostic Value of Lactic Acid in Patients with Acute Decompensated Heart Failure

Abstract：Objective: To investigate the effect of serum lactate levels on in-hospital adverse outcomes in patients with acute decompensated heart failure (ADHF). Methods: A retrospective study was conducted on 107 ADHF patients admitted to the Cardiac Critical Care Unit (CCU) of The First Affiliated Hospital of Xinxiang Medical University from January 2022 to January 2023. Patients were divided into two groups based on whether they experienced adverse outcomes: the adverse outcome group and the favorable outcome group. Univariate analysis and multivariate logistic regression analysis were used to identify risk factors for adverse outcomes in ADHF patients. The receiver operating characteristic (ROC) curve was applied to evaluate the predictive value of lactate for in-hospital adverse outcomes in ADHF patients. Results: The incidence of in-hospital adverse outcomes in ADHF patients was 42.99% (46/107). Twenty-four patients (52.17%) in the adverse outcome group had hyperlactatemia, while 29 patients (47.54%) in the favorable outcome group had hyperlactatemia. The difference in the incidence of hyperlactatemia between the two groups was statistically significant (χ2=5.892, P<0.05). Multivariate logistic regression analysis showed that atrial fibrillation and elevated lactate levels were independent risk factors for in-hospital adverse outcomes in ADHF patients (P<0.05). The area under the ROC curve (AUC) for lactate in predicting in-hospital adverse outcomes in ADHF patients was 0.758 [95% CI (0.668, 0.848)], with an optimal cutoff value of 2.1 mmol/L, sensitivity of 92.4%, and specificity of 90.5%. Conclusion: Lactate is an important independent predictor of in-hospital adverse outcomes in ADHF patients.
