Analysis of the Degree of Cardiac Valve Calcification in Patients with Diabetic Nephropathy and Its Influencing Factors

Abstract:Objective: To explore the degree of cardiac valve calcification and its influencing factors in patients with diabetic nephropathy. Methods: A total of 350 patients diagnosed with diabetic nephropathy during hospitalization in the Department of Nephrology at the First Affiliated Hospital of Harbin Medical University from January 2015 to January 2021 were selected. According to urinary microalbumin excretion, the patients were divided into three groups: normal proteinuria group (102 cases), microalbuminuria group (100 cases), and macroalbuminuria group (148 cases). Clinical data, echocardiography, and serum biochemical parameters were collected. Multivariate logistic regression analysis was used to analyze the influencing factors of aortic valve and mitral valve calcification in patients with diabetic nephropathy. Results: The incidence of aortic valve calcification and mitral valve calcification in the macroalbuminuria group was higher than that in the microalbuminuria group (P < 0.05), and the incidence in the microalbuminuria group was higher than that in the normal proteinuria group (P < 0.05). The incidence of aortic valve calcification was higher than that of mitral valve calcification in patients with diabetic nephropathy. Logistic multivariate analysis showed that age, gender, hypertension, urinary microalbumin, and hemoglobin were influencing factors for aortic valve calcification in patients with diabetic nephropathy (P < 0.05); age was a risk factor for mitral valve calcification (P < 0.05), while bilirubin was a protective factor for mitral valve calcification (P < 0.05). Conclusion: The occurrence rates of aortic valve and mitral valve calcification are related to urinary microalbumin excretion; aortic valve and mitral valve calcification as auxiliary tests may provide a basis for the diagnosis and assessment of the severity of diabetic nephropathy.
