Abstract:Objective: To analyze the relationship between Toll-like receptor 4 (TLR4) gene polymorphism and ischemic cerebrovascular and cardiovascular disease (ICVD) in patients with type 2 diabetes mellitus (T2DM). Methods: A total of 243 T2DM patients who visited our hospital from June 2019 to June 2022 were selected, and followed up until June 2023. According to whether ICVD occurred, they were divided into the occurrence group and non-occurrence group. General data of the two groups were compared. Hardy-Weinberg equilibrium test for TLR4 gene polymorphism was performed in both groups. The polymorphisms at TLR4 gene loci rs10759930, rs10983755, rs11536889, and rs7873784 were compared between the two groups. The Cox regression model was used to analyze the influencing factors of ICVD in T2DM patients. Results: The incidence of ICVD in T2DM patients was 23.21%. The age of the occurrence group was higher than that of the non-occurrence group (P < 0.05). The proportion of hypertension, systolic blood pressure, glycated hemoglobin (HbA1c), and blood urea nitrogen (BUN) levels were higher in the occurrence group (P < 0.05), while the duration of T2DM was longer in the occurrence group (P < 0.05), and high-density lipoprotein cholesterol (HDL-C) levels were lower (P < 0.05). There was no statistically significant difference between the actual genotypes of TLR4 gene rs10759930, rs10983755, rs11536889, and rs7873784 and the predicted genotype frequencies according to the Hardy-Weinberg equilibrium law (P > 0.05). The frequency of GG genotype and G allele at TLR4 gene rs11536889 was higher in the occurrence group than in the non-occurrence group (P < 0.05). Age, hypertension, T2DM duration, HDL-C, and the GG genotype at TLR4 gene rs11536889 were risk factors for ICVD in T2DM patients (P < 0.05). Conclusion: Age, hypertension, T2DM duration, HDL-C, and the GG genotype at TLR4 gene rs11536889 are closely related to the occurrence of ICVD in T2DM patients.
