Abstract:Dilated cardiomyopathy (DCM) is a heterogeneous myocardial disease that can lead to heart failure, arrhythmia, and sudden death. Its pathogenesis is complex, with difficult clinical treatment and poor prognosis. According to traditional Chinese medicine, DCM belongs to a syndrome of fundamental deficiency and superficial excess, and imbalance between 'Yang transforms into qi and Yin forms matter' is the core pathogenesis of DCM. Autophagy is a lysosome-mediated degradation system that maintains normal cellular function and dynamic balance by clearing damaged organelles, aggregated proteins, and invading pathogens. Autophagy is widely involved in the occurrence and development of DCM. Therefore, based on the theory of 'Yang transforms into qi and Yin forms matter', by exploring the physiological and pathological mechanisms of autophagy and its relevance to the TCM concept of 'Yang transforms into qi and Yin forms matter', we aim to regulate autophagy levels using the treatment method of 'warming Yang and transforming turbidity' and clarify the microscopic mechanisms of treating this disease. Balancing Yin and Yang throughout the course of DCM to maintain internal environmental stability provides new insights and strategies for the prevention and treatment of DCM with traditional Chinese medicine.
