Delayed Enhancement of Cardiac MRI and Dynamic Follow up of Cardiac Function Parameters for the Prognosis of Patients with Dilated Cardiomyopathy

Abstract:Objective: To explore the prognostic value of cardiac magnetic resonance (CMR) in the follow-up of patients with dilated cardiomyopathy (DCM). Methods: A total of 63 patients diagnosed with DCM at The Third People's Hospital of Chongqing University from December 2017 to March 2023 were selected as the study subjects. The endpoint events were heart failure rehospitalization or all-cause mortality, and they were divided into the progressing group (n=26) and the stable group (n=37). Univariate and multivariate Cox regression analyses were used to identify the influencing factors of DCM progression. Statistical analysis was performed on the dynamic follow-up results of CMR cardiac function parameters in 23 patients. Results: Univariate Cox regression analysis showed that LVEDVI, LVESVI, and the severity of delayed enhancement were higher in the progressing group compared to the stable group. Multivariate Cox regression analysis indicated that widespread delayed enhancement (LGE) was an independent risk factor for the prognosis of DCM. The dynamic CMR follow-up data of 23 patients showed that the LVEF measured by CMR was lower in the progressing group than in the stable group, while LVEDV and LVESV were higher in the progressing group, with the differences increasing as the disease progressed. Conclusion: Widespread LGE is an independent risk factor for the prognosis of DCM. A progressive decline in LVEF and a progressive increase in LVEDVI and LVESVI measured by multiple CMR assessments indicate a poor prognosis for DCM.
