Abstract:Objective: To investigate the effect of colchicine on inflammatory levels and prognosis in patients with coronary heart disease combined with gout. Methods: A total of 192 patients hospitalized at our hospital from January 2012 to April 2022 due to coronary heart disease with acute gout attacks were selected as the study subjects. According to whether colchicine was used after acute gout attack, the patients were divided into colchicine group and control group. Clinical data, white blood cell count (WBC), neutrophil-to-lymphocyte ratio (NLR), high-sensitivity C-reactive protein (hs-CRP) and other inflammatory indicators, as well as low-density lipoprotein cholesterol (LDL-C) levels were compared between the two groups. The incidence of major composite endpoints and minor composite endpoints was compared between the two groups, and the impact of colchicine on clinical outcomes was analyzed using Kaplan-Meier survival curves. Results: There was a statistically significant difference in the dose and duration of colchicine application between the two groups (P < 0.05). The NLR and hs-CRP levels in the colchicine group were significantly lower than those in the control group (P < 0.05), while WBC was higher in the colchicine group and LDL-C was lower, but these differences were not statistically significant (P > 0.05). In the colchicine group, 12 cases (13.33%) experienced major composite endpoints and 8 cases (8.89%) experienced minor composite endpoints, both of which were significantly lower than those in the control group (P < 0.05). Kaplan-Meier survival curves and Log-Rank test showed that colchicine could improve the clinical outcomes of patients with coronary heart disease combined with gout. Conclusion: Long-term use of colchicine in patients with acute gout attacks combined with coronary heart disease is beneficial in reducing systemic inflammatory response levels and can significantly reduce the incidence of adverse clinical outcomes such as acute myocardial infarction and heart failure.
