Prescription Rule and Mechanism of TCM Treatment for Heart Failure with Reduced Ejection Fraction Based on Data Mining and Network Pharmacology

Abstract:Objective:To analyze the prescription rule and mechanism of traditional Chinese medicine(TCM)in treating heart failure with reduced ejection fraction(HFrEF).Methods:Randomized controlled clinical trials of TCM treatment of HFrEF in electronic databases were searched systematically,and the potential mechanism of action of core prescriptions in the treatment of HFrEF was explored based on data mining and network pharmacology.Results:A total of 442 effective prescriptions were collected,involving 217 kinds of Chinese medicines,with a total frequency of 4 442.The core prescriptions include Astragalus membranaceus,Salvia miltiorrhiza,cassia twig,monkshood,radix glycyrrhizae,semen lepidii,and atractylodes macrocephala koidz.The results of the cluster analysis showed that the efficacy of the drug was concentrated in supplementing Qi,warming Yang,activating blood circulation,promoting diuresis and nourishing Yin.A total of 137 common targets of core prescription active ingredients and diseases were obtained,among which the top 10 of them were TP53,signal transducer and activator of transcription 3(STAT3),protein kinase B(AKT)1,interleukin(IL)6,tumor necrosis factor(TNF),mitogen activated protein kinase(MAPK)3,MAPK1,IL1β,estrogen receptor 1(ESR1),and transcription factor p65(RELA).The main enriched pathways included advanced glycation end products(AGE)-receptor for AGE(RAGE)signaling pathway in diabetic complications,lipid and atherosclerosis,and cancer pathways.Conclusion:The TCM prescription for HFrEF is based on Qi-tonifying drugs,blood-activating drugs,and diuresis-inducing drugs.Then,according to clinical symptoms,Yang-warming and Yin-nourishing drugs are used to assist in the treatment.
