Analysis of the Clinical Characteristics of Patients with Reverse Arytenoid Hypertension Complicated with Coronary Heart Disease

Abstract:Objective: To explore the clinical characteristics of patients with reverse dipping hypertension combined with coronary heart disease. Methods: A total of 900 patients with hypertension combined with coronary heart disease who were hospitalized in the Department of Cardiology at Qingdao Traditional Chinese Medicine Hospital from December 2021 to December 2023 were selected and divided into three groups: dipper group (170 cases), non-dipper group (408 cases), and reverse dipper group (322 cases). General clinical data, related biochemical tests, echocardiography, and ambulatory electrocardiogram data were collected. Results: The reverse dipper group had significantly higher age, N-terminal pro-B-type natriuretic peptide (NT-proBNP), percentage of PNN50 (>50 ms) in 24-hour adjacent sinus R-R interval differences, and left ventricular end-diastolic diameter (LVEDD) compared to the dipper and non-dipper groups, while the 24-hour standard deviation of all sinus R-R intervals (SDNN) was lower than that of the dipper and non-dipper groups, with statistically significant differences (P < 0.05). The occurrence of reverse dipping hypertension was positively correlated with age, nocturnal systolic blood pressure (NSBP), nocturnal diastolic blood pressure (NDBP), NT-proBNP, D-dimer, and creatine kinase isoenzyme (CK-MB) (P < 0.05), and negatively correlated with SDNN and daytime diastolic blood pressure (DDBP) (P < 0.05). Conclusion: Patients with reverse dipping hypertension show more pronounced loss of normal circadian rhythm, and there are significant differences in age, NT-proBNP, PNN50, LVEDD, and SDNN between reverse dipping, dipper, and non-dipper patients.
