The Relationship between Consistency/Inconsistency of LDL-C/ApoB and Recurrent Cardiovascular Events in Patients with Acute Myocardial Infarction after Lipid-lowering Therapy

Abstract:Objective: To observe the relationship between consistent/inconsistent levels of low-density lipoprotein cholesterol (LDL-C) and apolipoprotein B (ApoB) after lipid-lowering therapy in patients with acute myocardial infarction and recurrent cardiovascular events. Methods: A total of 233 patients with acute myocardial infarction who underwent percutaneous coronary intervention (PCI) at the Department of Cardiology, Cangzhou People's Hospital from January 2020 to January 2023 and met the criteria for very high-risk populations were selected as the study subjects. All patients had their lipid levels rechecked three months after discharge. According to the target values for lipid-lowering therapy in the 2023 Chinese Guideline for the Management of Dyslipidemia, the study subjects were divided into four groups: low LDL-C/low ApoB group, low LDL-C/high ApoB group, high LDL-C/low ApoB group, and high LDL-C/high ApoB group. Inconsistency was defined as LDL-C ≥1.40 mmol/L and ApoB <0.80 mmol/L, or vice versa. Clinical data and follow-up results of the four groups were compared, and survival analysis was used to evaluate the recurrence rate of cardiovascular events in the four groups. Results: There were statistically significant differences among the four groups in alanine aminotransferase, total bilirubin, total cholesterol, LDL-C, high-density lipoprotein cholesterol, ApoB, lipoprotein(a), and the recurrence rate of cardiovascular events (P < 0.05). After three months of lipid-lowering treatment, 44.21% (103/233) of patients achieved LDL-C targets, and 21.89% (51/233) of patients showed inconsistent LDL-C/ApoB levels. During a follow-up period of (25.05 ± 9.50) months, 19.74% (46/233) of patients experienced recurrent cardiovascular events. Kaplan-Meier survival curves and Log-Rank tests showed statistically significant differences in the recurrence rates of cardiovascular events among the four groups (χ2 = 21.458, P < 0.001). Conclusion: Consistency or inconsistency between LDL-C and ApoB can affect the risk of recurrent cardiovascular events in patients with acute myocardial infarction after PCI.
