The Correlation of miRNA Expression with Myocardial Infarction Type and Cardiomyocyte Apoptosis

Abstract:Objective: To analyze the correlation between microRNA (miRNA) expression and myocardial infarction types and cardiomyocyte apoptosis. Methods: A total of 60 patients with non-ST-segment elevation myocardial infarction (NSTEMI) who were treated at our hospital from January 2022 to June 2023 were selected as the NSTEMI group, 60 patients with ST-segment elevation myocardial infarction (STEMI) as the STEMI group, and 60 patients with stable angina pectoris as the control group. General information, laboratory data, and echocardiographic data of the patients were collected; blood samples were taken to detect miRNA expression. The correlation between the expression levels of various miRNAs and the type of myocardial infarction, myocardial enzymes, echocardiographic findings, and other indicators was analyzed. Results: Six hours after onset, the levels of creatine kinase isoenzyme (CK-MB) and troponin T (cTnT) in the control group were lower than those in the STEMI and NSTEMI groups, while the left ventricular ejection fraction (LVEF) and left ventricular end-diastolic volume (LVEDV) were higher than those in the STEMI and NSTEMI groups. The CK-MB and cTnT levels in the STEMI group were higher than those in the NSTEMI group, while LVEF and LVEDV were lower than those in the NSTEMI group (P < 0.05). The levels of miRNA-1, miRNA-24, miRNA-133a, miRNA-143, miRNA-152-5p, miRNA-203, and miRNA-204 in the control group were higher than those in the STEMI and NSTEMI groups, while miRNA-16, miRNA-21, and miRNA-3681-5p were lower than those in the STEMI and NSTEMI groups, with statistically significant differences (P < 0.05); the level of miRNA-3681-5p in the STEMI group was higher than that in the NSTEMI group, while miRNA-133a, miRNA-152-5p, miRNA-203, and miRNA-204 were lower than those in the NSTEMI group, with statistically significant differences (P < 0.05). Pearson correlation analysis showed that miRNA-133a was negatively correlated with cTnT and positively correlated with LVEF; miRNA-152-5p was negatively correlated with cTnT and LVEDV; miRNA-203 was negatively correlated with cTnT; miRNA-204 was positively correlated with LVEF; and miRNA-3681-5p was positively correlated with cTnT and LVEDV (P < 0.05). Conclusion: miRNA-133a, miRNA-152-5p, miRNA-203, miRNA-204, and miRNA-3681-5p are associated with myocardial infarction types, myocardial injury, and changes in cardiac structure to varying degrees.
