Abstract:Objective: To analyze the risk factors for contrast-induced nephropathy (CIN) after percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS), and to construct a nomogram predictive model. Methods: A retrospective analysis was conducted on the clinical data of 231 patients who underwent PCI at the Lu'an People's Hospital from 2019 to 2022. The patients were divided into CIN group (25 cases) and non-CIN group (206 cases) based on whether CIN occurred. Univariate analysis and logistic regression analysis were used to identify independent risk factors for CIN after PCI in ACS patients. Based on these factors, a nomogram predictive model was constructed and validated for clinical utility. Receiver operating characteristic (ROC) curves and decision curve analysis (DCA) were used to evaluate the clinical value of the model. Results: Univariate analysis showed that age, hypertension, diabetes, inflammatory markers [neutrophil count (N), lymphocyte count (L), neutrophil-to-lymphocyte ratio (NLR)], serum creatinine, left ventricular ejection fraction (LVEF) <40%, and estimated glomerular filtration rate (eGFR) <60 mL/(min·1.73 m²) were risk factors for CIN (P < 0.05). Logistic regression analysis indicated that age, NLR, serum creatinine, and eGFR <60 mL/(min·1.73 m²) were independent predictors of CIN (P < 0.05). A nomogram model was built using these four independent predictors. The ROC curve of the model showed an area under the curve (AUC) of 0.897, 95% CI [0.836, 0.958], a Youden index of 0.635, sensitivity of 0.960, and specificity of 0.775, indicating good predictive performance. The DCA curve showed a threshold probability range of 0 to 0.95, and the model demonstrated positive net benefit. Conclusion: The model constructed based on risk factors for CIN after PCI in ACS patients has good predictive value.
