Effect and Safety of Zhigancao Decoction Combined with Metoprolol for the Treatment of Coronary Heart Disease and Arrhythmia:a Systematic Review

Abstract：Objective:To evaluate the effect and safety of classical formula Zhigancao decoction combined with metoprolol for the treatment of coronary heart disease and arrhythmia.Methods:Randomized controlled trials for the treatment of coronary heart disease and arrhythmia with Zhigancao decoction combined with metoprolol were searched by China National Knowledge Infrastructure(CNKI),WanFang,VIP,China Biomedical Literature Service System(SinoMed),PubMed,Web of Science,and the Cochrane Library from the establishment of the database to June 2023.Two researchers independently screened the literatures,data extraction and quality evaluation.Review Mannager 5.3 software was used for Meta-analysis.Results:A total of 33 literatures were included,including 3 545 patients.The results of Meta-analysis showed that compared with the test group,the total clinical efficacy in the control group(RR=0.81,95％CI 0.79-0.84,P＜0.000 01)was lower,the heart rate(MD=5.72,95％CI 1.20-10.24,P=0.01),the QT interval dispersion(MD=9.92,95％CI 8.49-11.34,P＜0.000 01)lengthened,triacylglycerol(MD=0.65,95％CI 0.55-0.74,P＜0.000 01),low-density lipoprotein cholesterol(MD=0.50,95％CI 0.06-0.94,P=0.03),total cholesterol(MD=1.12 95％CI 0.99-1.25,P＜0.000 01)increased,high-density lipoprotein cholesterol(MD=-0.41,95％CI-0.51--0.31,P＜0.000 01)decreased.The occurrence frequency of atrial premature contractions(MD=214.33,95％CI 192.44-236.22,P＜0.000 01),ventricular premature contractions(MD=332.85,95％CI 317.48-348.22,P＜0.000 01),atroventricular junctional premature contractions(MD=61.32,95％CI 58.69-63.95,P＜0.000 01),and the incidence of adverse reactions(RR=3.18,95％CI 2.41-4.19,P＜0.000 01)increased.There were statistically significant differences.Conclusion:The current evidence showed that Zhigancao decoction combined with metoprolol for the treatment of coronary heart disease and arrhythmia was safe and effective.
