Study on the Influencing Factors of Symptom Clusters in Patients with Acute ST-segment Elevation Myocardial Infarction and the Timeliness of Treatment

Abstract:Objective: To explore the symptom clusters of patients with acute ST-segment elevation myocardial infarction (STEMI), analyze their influencing factors, investigate the current status of treatment timeliness, and analyze the relationship between symptom clusters and treatment timeliness, providing a reference for symptom cluster management and reducing the time to seek medical care. Methods: A convenience sampling method was used to select 136 STEMI patients who underwent percutaneous coronary intervention (PCI) at a tertiary general hospital in Xianyang City from January 2023 to September 2023 as the study subjects. Within 3 days of admission, a general information questionnaire and a disease perception questionnaire were used for a cross-sectional survey. The second-order clustering algorithm was used to extract symptom clusters, and logistic regression analysis was performed to identify the influencing factors. Results: Three symptom clusters were identified in STEMI patients: typical chest symptom cluster, multiple symptoms cluster, and atypical symptom cluster. Logistic multivariate regression analysis showed that gender [OR=0.172, 95% CI (0.052, 0.567), P=0.004] and admission method [OR=0.187, 95% CI (0.038, 0.927), P=0.040] were independent influencing factors of symptom clusters. Female patients were more likely to present with multiple symptom clusters, while patients with typical chest symptom clusters tended to choose to call the "120" ambulance for admission. In terms of medical treatment time, the FMC2W (first medical contact to wire crossing) time was shorter in the typical chest symptom cluster group, and the FMC2W compliance rate was higher, with statistically significant differences (P<0.05). Conclusion: STEMI patients have multiple symptom clusters, and different symptom clusters are related to patient gender, admission method, and treatment delay. Medical staff should carry out public education on myocardial infarction knowledge and health promotion to enable early identification of myocardial infarction symptoms and reduce the time to seek medical care.
