Analyse of the Degree of Premature Vascular Failure in Young and Middle-aged Patients with Different Blood Pressure based on the Brachial-ankle Pulse Wave Velocity

Abstract:Objective: To define vascular aging as a brachial-ankle pulse wave velocity (baPWV) ≥ 1400 cm/s, this study explores the degree of early vascular aging in young and middle-aged individuals with different blood pressure levels, providing new insights for the early diagnosis and treatment of cardiovascular diseases. Methods: A retrospective analysis was conducted on 3232 young and middle-aged patients who underwent baPWV testing at our hospital between November 2019 and October 2022. These patients were divided into three groups based on their blood pressure: normal blood pressure group (1131 patients), high-normal blood pressure group (1352 patients), and hypertension group (749 patients). The analysis included gender, age, body mass index (BMI), blood pressure, heart rate, baPWV, arterial stiffness, and complications. Results: The hypertension group had the youngest age and the highest BMI (P < 0.01). As blood pressure increased, baPWV gradually accelerated, and the proportion of middle-aged patients with baPWV ≥ 1400 cm/s was higher than that of young patients. Logistic regression analysis showed that age, gender, and blood pressure were independent risk factors for arterial stiffness (P < 0.01). The incidence of complications such as coronary heart disease, cerebral infarction, and carotid artery atherosclerosis increased with rising blood pressure. Moreover, in the high-normal blood pressure group and hypertension group, patients with baPWV ≥ 1400 cm/s had higher rates of coronary heart disease, cerebral infarction, and carotid artery atherosclerosis compared to those with baPWV < 1400 cm/s (P < 0.01). Conclusion: Among young and middle-aged patients, those with hypertension are younger and have more severe vascular damage than those with normal blood pressure. baPWV can predict the occurrence of cardiovascular and cerebrovascular complications in individuals with high-normal blood pressure and hypertension.
