Value of Point-of-care Testing of Myocardial Biochemical Markers in Diagnosis and Short-term Prognosis of Patients with Acute non-ST Segment Elevation Acute Coronary Syndrome

Abstract:Objective: To explore the predictive value of bedside rapid detection (POCT) of serum creatine kinase isoenzyme (CK-MB), myoglobin (MYO), and cardiac troponin I (cTnI) levels in the diagnosis and short-term prognosis of patients with acute non-ST-segment elevation acute coronary syndrome (NSTE-ACS). Methods: A total of 110 patients with acute heart disease who presented to the emergency department of Mianyang Third People's Hospital between July 2020 and March 2022, with non-ST-segment elevation on electrocardiogram, were selected. The levels of serum CK-MB, MYO, and cTnI were detected using POCT, and the patients were divided into an observation group (NSTE-ACS) and a control group (non-NSTE-ACS) based on the final diagnosis. The diagnostic value of POCT-detected cTnI, CK-MB, and MYO for NSTE-ACS was analyzed. A binary logistic regression model was used to analyze the influencing factors of NSTE-ACS. Complications and mortality within 30 days of follow-up were observed. Results: There were statistically significant differences in cTnI and CK-MB levels between the two groups (P < 0.05), but no significant difference in MYO levels (P > 0.05). cTnI, CK-MB, and MYO had some diagnostic efficacy for NSTE-ACS, but their diagnostic performance was poor, with the area under the receiver operating characteristic (ROC) curve (AUC) for all three markers being less than 0.7. Gender, age, cTnI, CK-MB, MYO, and GRACE score had no significant effect on NSTE-ACS (P > 0.05). Logistic regression analysis showed that cTnI, CK-MB, and MYO had predictive value for the short-term prognosis of NSTE-ACS patients. There were no statistically significant differences in the incidence of complications and mortality within 30 days of follow-up between the two groups (P > 0.05). Conclusion: POCT has a certain predictive value for the short-term prognosis of patients with acute NSTE-ACS, but the predictive value is not high. Further exploration of other biomarkers or combined testing is needed to improve diagnostic accuracy and predict patient prognosis.
