Exploration of Syndrome and Treatment Rule of Modified Baoyuan Yangxin Formula for the Treatment of Ventricular Precontraction Based on the Study Model of Syndrome-symptom-medication-prescription

Abstract：Objective:Using the method of retrospective case study and data mining technology,this study explored the treatment rule of Baoyuan Yangxin Formula by professor Yuan Haibo for the treatment of ventricular precontraction(PVCs).Methods:Outpatient cases treated with modified Baoyuan Yangxin Formula for ventricular precontraction in the medical record system of the First Affiliated Hospital of Henan University of Chinese Medicine from December 2020 to December 2022 were collected and recorded into the V3.0 software of TCM inheritance computing platform,and the rule of syndrome and treatment was studied with the software.Results:A total of 203 cases were collected,and there were 4 common traditional Chinese medicine(TCM)syndrome types,including Qi Yin deficiency and blood stasis syndrome,Qi Yin deficiency and blood stasis syndrome with Qi stagnation,Qi Yin deficiency and blood stasis syndrome with phlegm dampness,and Qi Yin deficiency and blood stasis syndrome with Yang deficiency.Overall research showed that palpitations,insomnia,fatigue,chest tightness,shortness of breath,restlessness,dry mouth,and chest pain were the high-frequency symptoms of Baoyuan Yangxin Formula for the treatment of PVCs.The tongue was dull,the veins were blue and purple,the coating was thin and white,and the veins were thin,which were high-frequency tongue veins.The commonly used formula for treating PVCs was the Baoyuan Yangxin Formula,which was a combination of high-frequency drugs such as Radix Pseudostellariae,Poria cocos,roasted licorice,Salvia miltiorrhiza,sour jujube kernel,Atractylodes macrocephala,Xu Changqing,and amethyst.Four types of new prescriptions were identified through drug cluster analysis.Research on syndrome differentiation showed that,palpitations,insomnia,dry mouth,restlessness,fatigue,dizziness,etc.were the high-frequency symptoms and combinations of Qi Yin deficiency and blood stasis syndrome in PVCs.Red tongue texture,blue and purple veins,thin and white fur,and thin veins were high-frequency tongue veins.The addition and subtraction of Baoyuan Yangxin Formula were commonly used formulas.Palpitation,palpitations,chest tightness,emotional distress,insomnia,restlessness,fatigue,and hypochondriasis were high-frequency symptoms and Qi Yin deficiency and blood stasis combined with Qi stagnation in PVCs.The tongue was dull,the veins were blue and purple,the coating was thin and white,and the veins were thin,which were high-frequency tongue veins.Baoyuan Yangxin Formula with modified Chaihu Shugan Powder was a commonly used formula.Palpitation,palpitations,fatigue,bloating in the chest and abdomen,sticky mouth,pantothenic acid,numbness,and sticky stools were the high-frequency symptoms and combinations of Qi Yin deficiency and blood stasis syndrome with phlegm dampness in PVCs.The tongue texture was dark red,the coating was thin,white,and greasy,the collaterals were blue and purple,and the pulse strings were high-frequency tongue veins Bao Yuan Yang Xin Fang plus or minus Liu Junzi Tang was a commonly used formula.Palpitation,dizziness,fatigue,cold limbs,lower limb swelling,chest tightness,and shortness of breath were high-frequency symptoms and combinations of both Qi and Yin deficiency and Yang deficiency in PVCs.Fat tongue,white and smooth fur,blue and purple veins,and thin veins were high-frequency tongue veins.Baoyuan Yangxin Decoction plus or minus Guizhi Licorice Longgu Oyster Decoction and Tingli Dazao Xiefei Decoction were commonly used.Conclusion:Qi Yin deficiency and blood stasis syndrome is the basic syndrome type for the treatment of PVCs with modified Baoyuan Yangxin Formula.It can also present three types of syndromes,Qi stagnation,phlegm dampness,and Yang deficiency.In terms of treatment,medication is based on supplementing Qi,nourishing Yin,and promoting blood circulation.According to the different types of symptoms and syndromes,soothing the liver and regulating Qi,eliminating dampness and phlegm,and warming Yang and promoting diuresis are given separately.
