The Clinical Value of Residual Lipoprotein Cholesterol and Non-high-density Lipoprotein Cholesterol in Predicting the Risk of Coronary Heart Disease in Elderly Patients

Abstract:Objective: To explore the clinical value of remnant lipoprotein cholesterol (RLP-C) and non-high-density lipoprotein cholesterol (non-HDL-C) in predicting the risk of coronary heart disease (CHD) in the elderly. Methods: A retrospective study was conducted, selecting patients with coronary heart disease and healthy individuals undergoing physical examinations at the Department of Cardiology, Cangzhou People's Hospital, as the study subjects. The CHD group consisted of 401 patients, while the control group included 362 healthy individuals. Clinical data, medical history, and laboratory test indicators were compared between the two groups. Logistic regression models were used to analyze the risk factors for coronary heart disease in the elderly, and receiver operating characteristic (ROC) curves were drawn to evaluate the predictive value of RLP-C and non-HDL-C for coronary heart disease. Results: There were statistically significant differences between the two groups in terms of smoking rates, hypertension rates, diabetes rates, hyperuricemia rates, body mass index (BMI), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), RLP-C, and non-HDL-C (P < 0.05). Multivariate logistic regression analysis showed that smoking [OR = 1.459, 95% CI (1.015, 2.097), P = 0.041], BMI [OR = 1.152, 95% CI (1.102, 1.204), P < 0.001], hypertension [OR = 1.399, 95% CI (1.018, 1.923), P = 0.038], diabetes [OR = 1.567, 95% CI (1.052, 2.334), P = 0.027], hyperuricemia [OR = 2.090, 95% CI (1.295, 3.371), P = 0.003], RLP-C [OR = 1.605, 95% CI (1.224, 2.104), P = 0.001], non-HDL-C [OR = 3.510, 95% CI (2.630, 4.683), P < 0.001], TC [OR = 2.495, 95% CI (2.032, 3.063), P < 0.001], and LDL-C [OR = 1.352, 95% CI (1.109, 1.648), P = 0.003] were independent risk factors for coronary heart disease, while HDL-C [OR = 0.043, 95% CI (0.020, 0.092), P < 0.001] was a protective factor. The areas under the ROC curve for RLP-C and non-HDL-C in predicting the risk of coronary heart disease were 0.679 [95% CI (0.641, 0.717), P < 0.001] and 0.733 [95% CI (0.698, 0.768), P < 0.001], respectively. Conclusion: RLP-C and non-HDL-C have good predictive effects on coronary heart disease in elderly patients.
