Meta-analysis and GRADE Evidence Evaluation on the Intervention of Oral Chinese Patent Medicine in Coronary Slow Flow Phenomenon

Abstract：Objective:To evaluate the efficacy and safety of oral Chinese patent medicine in the intervention of coronary slow flow phenomenon(CSFP).Methods:Randomized controlled trials of oral Chinese patent medicine intervention for the treatment of CSFP were searched from CNKI,WanFang,VIP,China Biomedical Literature Database,PubMed,EMbase,the Cochrane Library,and Wed of Science from the establishment of the database to December 1,2022.The quality of literature was evaluated by Cochrane bias risk assessment tool and modified Jadad scale.RevMan 5.3 and Stata 17.0 were used for Meta-analysis to evaluate primary outcome measures[corrected thrombolysis in myocardial infarction frame count(cTFC),incidence of grade 3 blood flow of thrombolysis in myocardial infarction(TIMI),index of coronary microvascular resistance(IMR)],secondary outcome measures[hypersensitive C-reactive protein(hs-CRP),Seattle Angina Pectoris Scale(SAQ)score]and incidence of adverse reactions.GRADEpro GDT was used to evaluate the quality of evidence.Results:A total of 20 literatures were included,including 1 417 patients.The quality of the literature included in the study was generally low.The results of Meta-analysis showed that compared with the control group,cTFC(MD=-8.57,95%CI-12.60--4.54,P＜0.000 1),IMR(MD=-4.15,95%CI-5.60--2.69,P＜0.000 01)and hs-CRP(MD=-1.57,95%CI-1.81--1.33,P＜0.000 01)decreased in the experimental group,the incidence of TIMI blood flow grade 3(RR=2.04,95%CI 1.66-2.50,P＜0.000 01)and SAQ score(MD=67.93,95%CI 54.24-81.63,P＜0.000 01)increased in the experimental group.In terms of safety,there was no significant difference in the incidence of adverse reactions between two groups(P＞0.05).GRADE showed that the incidence of TIMI blood flow grade 3 was intermediate,the incidence of cTFC,hs-CRP,SAQ score and adverse reactions was low,and the incidence of IMR was extremely low.Conclusion:The current evidence showed that compared with conventional western medicine alone,oral Chinese patent medicine can further improve the coronary blood flow,coronary microvascular function and quality of life of CSFP patients,and inhibit the inflammatory response.
