Analysis of Serum ADAMTS13 and TP53 Expression Levels and Prognostic Value in Patients with Chronic Heart Failure

Abstract:Objective: To investigate the expression levels of ADAMTS13 and TP53 in serum of patients with chronic heart failure (CHF) and their clinical value in prognosis evaluation. Methods: A total of 140 CHF patients hospitalized at our hospital from January 2020 to June 2021 were selected as the CHF group, and were divided into NYHA class II (52 cases), III (46 cases), and IV (42 cases) according to the New York Heart Association (NYHA) functional classification. All CHF patients were followed up for one year by telephone and outpatient review. Patients who experienced recurrence or death were considered as having poor prognosis and were divided into the good prognosis group (84 cases) and poor prognosis group (56 cases). Another 140 healthy individuals undergoing physical examination during the same period were selected as the control group. The serum levels of ADAMTS13 and TP53 were compared among the groups. Logistic regression analysis was used to analyze the factors affecting the prognosis of CHF patients. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the predictive value of serum ADAMTS13 and TP53 levels for the prognosis of CHF patients. Results: Compared with the control group, the serum ADAMTS13 level was significantly decreased, while the TP53 level was significantly increased in the CHF group (P < 0.05). With the increase in NYHA functional classification, the serum ADAMTS13 expression level gradually decreased, while the TP53 expression level gradually increased (P < 0.05). The NT-proBNP, LVEDD, and TP53 levels were significantly higher in the poor prognosis group than in the good prognosis group, while the LVEF and ADAMTS13 levels were significantly lower (P < 0.05). Multivariate logistic regression analysis showed that serum ADAMTS13, TP53, NT-proBNP, LVEF, and LVEDD were factors affecting the prognosis of CHF patients (P < 0.05). The area under the curve (AUC) for the combined prediction of CHF prognosis by ADAMTS13 and TP53 was 0.905, with a sensitivity of 83.93% and a specificity of 86.90%. Conclusion: The serum ADAMTS13 level is significantly reduced, and the TP53 level is significantly increased in patients with CHF. Combined detection of ADAMTS13 and TP53 has good reference value for the prognosis of CHF patients.
