Abstract:Objective: To explore the relationship between serum chitinase-3-like protein 1 (CHI3L1), nephroaminase and cognitive dysfunction in elderly patients with chronic heart failure (CHF). Methods: A total of 114 CHF patients admitted to our hospital from July 2021 to July 2022 were selected as the CHF group. According to the New York Heart Association (NYHA) functional classification, the CHF patients were divided into the II grade group (39 cases), III grade group (40 cases), and IV grade group (35 cases). At the same time, 114 healthy volunteers who underwent physical examination in our hospital were selected as the control group. The cognitive function of CHF patients was assessed by the Montreal Cognitive Assessment (MoCA) scale, and the CHF patients were divided into the normal group (54 cases) and the impairment group (60 cases). The levels of serum CHI3L1 and nephroaminase were detected by enzyme-linked immunosorbent assay (ELISA). Receiver operating characteristic (ROC) curve analysis was used to evaluate the diagnostic value of serum CHI3L1 and nephroaminase for cognitive dysfunction in CHF patients. Logistic regression analysis was performed to identify the influencing factors of cognitive dysfunction in CHF patients. Results: Compared with the control group, the serum CHI3L1 level was elevated, while the nephroaminase level was decreased in the CHF group (P < 0.05). The serum CHI3L1 levels increased progressively in the II grade group, III grade group, and IV grade group, while the nephroaminase levels decreased progressively (P < 0.05). Compared with the normal group, the serum CHI3L1 level was higher, and the nephroaminase level was lower in the impairment group (P < 0.05). ROC analysis showed that the area under the curve (AUC) of CHI3L1 combined with nephroaminase for predicting cognitive dysfunction in CHF patients was higher than that of CHI3L1 or nephroaminase alone (Z values were 7.347 and 11.640, respectively, P < 0.05). Logistic regression analysis results showed that the levels of CHI3L1 and nephroaminase were risk factors for cognitive dysfunction in CHF patients (P < 0.05). Conclusion: CHI3L1 is highly expressed in the serum of elderly CHF patients, while nephroaminase is lowly expressed. Both are related to cognitive function and may serve as diagnostic indicators for cognitive dysfunction in CHF patients.
