Effect of Dapagliflozin on Structural Remodeling and Oxidative Stress in Patients with HFrEF Complicated with Persistent Atrial Fibrillation after Radiofrequency Ablation

Abstract:Objective: To investigate the effect of sodium-glucose cotransporter 2 inhibitor (SGLT-2i) dapagliflozin on structural remodeling and oxidative stress levels in patients with heart failure with reduced ejection fraction (HFrEF) combined with persistent atrial fibrillation after radiofrequency ablation. Methods: A total of 124 patients with persistent atrial fibrillation combined with HFrEF who were treated at the Department of Cardiology, Zhengzhou Cardiovascular Hospital from October 2020 to March 2022 were selected as the study subjects. According to the random number table method, they were divided into the observation group (63 cases) and the control group (61 cases). Both groups received 50 W power radiofrequency ablation treatment and standard heart failure treatment before enrollment. The observation group was additionally given dapagliflozin tablets for a continuous treatment of 6 months. The therapeutic effects after 6 months were compared between the two groups. Results: After treatment, the left ventricular ejection fraction (LVEF), 6-minute walk test distance (6MWD), and sinus rhythm ratio in the observation group were higher than those in the control group, while the left atrial diameter (LAD), left ventricular diameter (LVDd), and heart rate were lower than those in the control group (P < 0.05). The body mass index (BMI), low-density lipoprotein cholesterol (LDL-C), epicardial adipose tissue (EAT) volume, and uric acid level in the observation group were lower than those in the control group (P < 0.05). The serum levels of high-sensitivity C-reactive protein (hs-CRP), N-terminal pro-B-type natriuretic peptide (NT-proBNP), reactive oxygen species (ROS), and superoxide dismutase (SOD) in the observation group were lower than those in the control group (P < 0.05). The incidence of adverse cardiovascular events in the observation group was lower than that in the control group (P < 0.05). Conclusion: Dapagliflozin can effectively improve cardiac structural remodeling in patients with HFrEF combined with persistent atrial fibrillation after radiofrequency ablation, reduce uric acid levels, regulate lipid metabolism, alleviate EAT, and decrease the expression of oxidative stress, thereby reducing the occurrence of adverse cardiovascular events.
