Relatioship Between Prehospital Features and All-cause Death During Hospitalization in Patients with Acute Myocardial Infarction and Cardiogenic Shock After PCI

Abstract:Objective: To explore the impact of prehospital characteristics on all-cause mortality during hospitalization in patients with acute myocardial infarction (AMI) complicated by cardiogenic shock (CS) who underwent successful percutaneous coronary intervention (PCI). Methods: A total of 97 patients with AMI complicated by CS who were successfully resuscitated prehospital and underwent successful PCI between September 1, 2019, and September 1, 2021, were selected. Their basic characteristics, prehospital features, and in-hospital features were analyzed, and factors influencing all-cause mortality during hospitalization were identified. Results: Among the 97 patients, the mean age was (58.07±11.03) years, with 63 males (64.95%). Twenty-three patients (23.71%) had non-ST-segment elevation myocardial infarction (NSTEMI), 18 (18.56%) were living alone, 32 (32.99%) were unemployed or retired, the total ischemia time was (6.35±2.20) hours, systolic blood pressure was (75.46±9.39) mmHg, the number of affected vessels was (1.54±0.74), with 14 (14.43%) having multi-vessel disease, the hospital stay was (16.98±6.17) days, and 22 (22.68%) died from any cause. Logistic analysis showed that being single-living, diabetes, total ischemia time, NSTEMI, combined use of vasopressive drugs, multi-vessel disease, and in-hospital complications were risk factors for all-cause mortality during hospitalization. Conclusion: Being single-living, total ischemia time, and combined use of vasopressive drugs are risk factors affecting all-cause mortality during hospitalization.
