Abstract:Objective: To investigate the effect of transversus abdominis plane block (TAPB) on adverse cardiovascular events (ACVE) in diabetic patients undergoing gynecological laparoscopic surgery. Methods: A total of 42 diabetic patients who underwent gynecological laparoscopic surgery at the Second Hospital of Shanxi Medical University from March to September 2022 were randomly divided into TAPB group and non-TAPB group, with 21 cases in each group. The TAPB group received TAPB under ultrasound guidance combined with general anesthesia. Heart rate, mean arterial pressure (MAP), and incidence of ACVE were compared at three time points: 30 minutes before surgery (T1), during skin incision (T2), and immediately after surgery (T3). Serum levels of neuropeptide Y (NPY) and cardiac troponin I (cTnI) were measured at T1 and 24 hours postoperatively (T4). Results: There was no statistically significant difference in MAP between the two groups at different time points (P > 0.05). At T2, heart rate in the TAPB group was lower than that in the non-TAPB group, with a statistically significant difference (P < 0.05). There was no statistically significant difference in the overall incidence of ACVE between the two groups (P > 0.05). At T4, serum NPY levels in both groups were lower than those in the non-TAPB group, with a statistically significant difference (P < 0.05). Compared with T1, NPY levels at T4 increased significantly in both groups, with TAPB group showing lower NPY levels than the non-TAPB group, and the differences were statistically significant (P < 0.05). Compared with T1, cTnI levels at T4 increased in the non-TAPB group, while cTnI levels in the TAPB group were lower than those in the non-TAPB group, with statistically significant differences (P < 0.05). Conclusion: TAPB may reduce sympathetic nervous system activity and stress response during the perioperative period, potentially offering cardioprotective effects.
