Tei Index and Free Triiodothyronine on Predicting Long-term Prognosis in Patients with Dilated Cardiomyopathy

Abstract:Objective: To explore the predictive value of Tei index combined with free triiodothyronine (FT3) for the long-term prognosis of patients with dilated cardiomyopathy (DCM). Methods: A total of 312 DCM patients hospitalized at Luoyang Central Hospital, Zhengzhou University from May 2016 to October 2018 were selected. After a follow-up period of (40.46±1.89) months, 93 cases (29.81%) experienced major adverse cardiovascular events (MACE). Patients were divided into MACE group (n=93) and non-MACE group (n=219). Cox regression analysis was used to identify factors influencing MACE during follow-up. Receiver operating characteristic (ROC) curves were applied to evaluate the predictive value of Tei index and FT3 for MACE during follow-up. Results: The NYHA heart function classification, left ventricular end-diastolic diameter (LVEDD), N-terminal pro-B-type natriuretic peptide (NT-proBNP), and Tei index were higher in the MACE group compared to the non-MACE group (P<0.05), while the application rate of renin-angiotensin-aldosterone system (RAAS) inhibitors and FT3 levels were lower in the MACE group (P<0.05). The higher the NYHA heart function classification, the higher the Tei index and the lower the FT3 level in DCM patients. Multivariate Cox regression analysis showed that Tei index [HR=1.039, 95%CI (1.026, 1.051)] and FT3 [HR=0.464, 95%CI (0.372, 0.578)] were independent risk factors for MACE during follow-up. The area under the curve (AUC) for predicting MACE was 0.700 for Tei index and 0.750 for FT3. The combination of Tei index and FT3 had better predictive value (AUC=0.809), with sensitivity of 75.3% and specificity of 73.1%. Patients with Tei index >0.66 and FT3 <3.39 pmol/L had a higher incidence of MACE (Log-Rank χ²=80.454, P<0.05). Conclusion: The combination of Tei index and FT3 has better predictive value for the prognosis of DCM. Patients with high Tei index and low FT3 have worse prognosis.
