The Changes of Serum sCD14-ST and Flt3L in Patients with Infective Endocarditis and Their Relationship between Prognosis

Abstract:Objective: To investigate the changes in serum soluble cluster of differentiation 14 subtype (sCD14-ST) and FMS-like tyrosine kinase 3 ligand (Flt3L) expression levels in patients with infective endocarditis (IE) and their relationship with prognosis. Methods: A total of 76 IE patients treated at Hainan Provincial People's Hospital from January 2019 to January 2023 were selected as the experimental group and divided into groups with good and poor prognosis based on their outcomes. A total of 42 healthy individuals undergoing physical examination during the same period were selected as the control group. The general inflammatory indicators and serum sCD14-ST and Flt3L levels were compared among the study subjects. Pearson correlation analysis was used to evaluate the correlation between serum sCD14-ST, Flt3L, and general inflammatory indicators. Receiver operating characteristic (ROC) curves were used to analyze the predictive value of sCD14-ST and Flt3L for the prognosis of IE patients. Results: Compared with the control group, the experimental group showed significantly increased levels of inflammatory indicators including interleukin-6 (IL-6), procalcitonin (PCT), high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor-alpha (TNF-α), white blood cell count (WBC), as well as serum sCD14-ST and Flt3L (P < 0.05). Pearson correlation analysis showed that sCD14-ST was positively correlated with PCT, hs-CRP, and WBC (P < 0.001); Flt3L was positively correlated with IL-6 and WBC (P < 0.001). The serum levels of sCD14-ST and Flt3L were higher in the poor prognosis group than in the good prognosis group (P < 0.01). ROC curve results showed that the area under the curve (AUC) for combined diagnosis of sCD14-ST and Flt3L was significantly greater than that for sCD14-ST alone (Z=2.627, P=0.009) or Flt3L alone (Z=2.575, P=0.010). Conclusion: Serum sCD14-ST and Flt3L levels are elevated in IE patients, and the combined detection of sCD14-ST and Flt3L levels can be applied in the clinical diagnosis of the prognosis of IE patients.
