Abstract:Objective: To explore the clinical efficacy of Shenfu Injection combined with Coenzyme Q10 in the treatment of acute carbon monoxide poisoning (ACOP) with myocardial injury, and to observe its effects on echocardiographic parameters and serum myocardial injury markers, tumor necrosis factor-α (TNF-α), total antioxidant capacity (T-AOC), and inducible nitric oxide synthase (iNOS) levels. Methods: A total of 116 patients with ACOP complicated with myocardial injury admitted to our hospital from January 2020 to January 2023 were randomly divided into an observation group and a control group, with 58 cases in each group. The observation group was treated with Shenfu Injection combined with Coenzyme Q10 sodium chloride injection, while the control group received only Coenzyme Q10 sodium chloride injection. After continuous treatment for 10 days, the clinical efficacy of both groups was observed. The echocardiographic parameters [left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), ratio of early diastolic mitral inflow velocity (E) to early diastolic mitral annular velocity (e') (E/e')], serum myocardial injury markers [cardiac troponin I (cTnI), creatine kinase isoenzyme (CK-MB), heart-type fatty acid-binding protein (H-FABP), N-terminal pro-B-type natriuretic peptide (NT-proBNP)], and levels of TNF-α, T-AOC, and iNOS were compared before and after treatment. Adverse reactions in both groups were also recorded. Results: The total effective rate of the observation group was 93.10% (54/58), which was higher than that of the control group at 77.59% (45/58), with a statistically significant difference (P < 0.05). After treatment, LVEDD, E/e', and serum levels of cTnI, CK-MB, H-FABP, NT-proBNP, TNF-α, and iNOS in both groups decreased compared to before treatment, while LVEF and serum T-AOC levels increased, with the observation group showing better results than the control group, and the differences were statistically significant (P < 0.05). Adverse reactions in both groups were few and mild. Conclusion: Shenfu Injection combined with Coenzyme Q10 can regulate serum myocardial injury markers and levels of TNF-α, T-AOC, and iNOS in patients with ACOP complicated with myocardial injury, improve cardiac function, and enhance clinical efficacy.
