The Relationship between Immediate D-dimer and Heart Failure in Patients with Acute Myocardial Infarction after Emergency Interventional Therapy

Abstract:Objective:To investigate the relationship between D-dimer(D-D)and heart failure after emergency interventional therapy in patients with acute myocardial infarction(AMI).Methods:One hundred and eighty-six patients with AMI from January 2020 to March 2022 were divided into D-D elevated group(100 cases)and D-D normal group(86 cases)according to the serum D-D level detected immediately.The propensity score matching(PSM)method was used to match the two groups according to 1∶1,with 70 cases in each group,and the general data of the two groups were compared.Restricted cubic spline Logistic regression model was used to analyze the dose-response relationship between D-D levels and heart failure risk.Multivariate Logistic regression was used to analyze the risk factors of heart failure.The patients were randomly divided into the training set(93 cases)and the validation set(93 cases)at a ratio of 1∶1.The artificial neural network(ANN)prediction model was constructed and evaluated.The clinical value of D-D level in predicting the occurrence of heart failure after emergency interventional therapy was evaluated by receiver operating characteristic(ROC)curve analysis.Results:Analysis with restricted cubic spline model showed that there was no non-linear relationship between D-D levels and heart failure risk in either men or women(P>0.05).Serum troponin I(cTnI),creatine kinase isoenzyme(CK-MB),B-type natriuretic peptide(BNP)and hypersensitive C-reactive protein(hs-CRP)levels increase and left ventricular ejection fraction(LVEF)decrease were the risk factors of heart failure(P<0.05).Among the importance scores of ANN variables,D-D,BNP,and LVEF showed a higher contribution to model classification.ROC curve showed that the model showed good differentiation and accuracy.Conclusion:The increase of D-D level in AMI patients could significantly increase the risk of heart failure after emergency interventional therapy.The detection of D-D level in AMI patients could be used for predicting the occurrence of heart failure after emergency interventional therapy.
