Relationship between Serum CCL17,CXCL8 and Cardiac Function,Prognosis in Elderly Patients of Heart Failure with Preserved Ejection Fraction

Abstract:Objective:To investigate the relationship between serum C-C chemokine ligand 17(CCL17),C-X-C chemokine ligand 8(CXCL8)and cardiac function,prognosis in elderly patients of heart failure with preserved ejection fraction(HFpEF).Methods:A total of 168 HFpEF patients(HFpEF group)and 81 volunteers with normal cardiac function(control group)were selected.Serum CCL17 and CXCL8 levels were detected by enzyme-linked immunosorbent assay.Left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),mitral valve peak blood flow(E)in early diastolic period and peak blood flow(A)in late diastolic period were measured by echocardiography.HFpEF patients were followed up after discharge,and the incidence of adverse cardiovascular events during follow-up was analyzed.Pearson correlation was used to describe the correlation between CCL17,CXCL8 and echocardiographic parameters.Multivariate Logistic regression was used to analyze the factors affecting the prognosis of elderly HFpEF patients.The clinical value of CCL17 and CXCL8 in predicting prognosis of elderly HFpEF patients was analyzed by receiver operating characteristic(ROC)curve.Results:Serum levels of CCL17 and CXCL8 in the HFpEF group were higher than those in the control group,and the differences were statistically significant(P<0.001).LVESD and LVEDD in the HFpEF group were higher than those in the control group,and LVEF and E/A ratio were lower than those in the control group,with statistical significance(P<0.001).Serum CCL17 and CXCL8 levels in HFpEF patients were positively correlated with LVESD and LVEDD(P<0.001),and negatively correlated with LVEF and E/A ratio(P<0.01).The levels of CCL17,CXCL8,NT-proBNP,LVESD and LVEDD in the poor prognosis group were higher than those in the better prognosis group(P<0.05),and LVEF and E/A ratio in the poor prognosis group were lower than those in the the better prognosis group(P<0.05).New York Heart Association(NYHA)heart function Ⅳ class,high CCL17,high CXCL8 and high NT-proBNP were the risk factors for poor prognosis in HFpEF patients(P<0.05).The area under ROC curve(AUC)of CCL17 and CXCL8 in predicting poor prognosis of elderly HFpEF patients was 0.776 and 0.800.The AUC of combined CCL17 and CXCL8 in predicting poor prognosis of elderly HFpEF patients was 0.879,which was greater than that of CCL17 and CXCL8 alone(Z value were 2.333,2.193,P<0.05).Conclusion:Serum levels of CCL17 and CXCL8 increased in elderly HFpEF patients,which were associated with reduced cardiac function and poor prognosis.The detection of serum CCL17 and CXCL8 could predict the risk of poor prognosis.
