Symptom-drug Correspondence and Mechanism of Traditional Chinese Medicine for the Treatment of Chronic Heart Failure Based on Symptom-drug Network and Network Pharmacology

Abstract:Objective:To explore the driving drugs and the symptom-drug relationship in the clinical cases of chronic heart failure(CHF),and to reveal the related mechanisms based on network pharmacology.Methods:The effective clinical cases of CHF with traditional Chinese medicine prescriptions were searched from CNKI,WanFang Data Knowledge Service platform and VIP databases.The FangNet platform was used to screen the driving drugs and construct the"symptom-drug"network for the four traditional Chinese medicine diagnosis information and traditional Chinese medicine prescription contained from these cases.The"drug-component-symptom-disease-target"network was constructed to reveal the relevant mechanism of drug intervention in disease symptoms by network pharmacology.Results:A total of 226 clinical cases were obtained,involving 268 flavors of drugs,23 of which were driving drugs.The"symptom-drug"network included 38 common CHF symptoms including lower limb swelling,edema,chest oppression,asthma,shortness of breath,and oliguria,which correspond to 10 driving Chinese medicines,including Poria cocos,Astragalus membranaceus,Atractylodes macrocephala,Radix et Rhizoma Polygoni Multiflori,Radix et Rhizoma Polygoni Multiflori,Radix et Rhizoma Polygoni Multiflori,Radix et Rhizoma Polygoni Multiflori and Radix et Rhizoma Polygoni.Through network pharmacology,a"drug-composition-symptom-symptom-disease-target"network corresponding to chest oppression in the"symptom-drug"network of salvia miltiorrhiza and semen lepidii was constructed,in which quercetin,luteolin,kaempferol and tanshinoneⅡA were selected as key components.G1/S phase specific cyclin-D1(CCND1),hypoxic inducible factor 1α(HIF1A),transcription factor Jun(JUN),protein kinase B1(AKT1),transcriptional activator 3(STAT3),cysteine aspartate proteinase-3(CASP3),Myc proto-oncoprotein(MYC),vascular endothelial growth factor A(VEGFA)were selected as key target.Enrichment analysis showed that the mechanism of salvia miltiorrhiza and semen lepidii to improve chest oppression in CHF patients might involve biological processes such as negative regulation of apoptosis signaling pathway,cyclin-dependent protein kinase enzyme complex,phosphatidylinosiol 3 kinase/protein kinase B(PI3K/AKT)signaling pathway,tumor suppressor gene p53 signaling pathway,epidermal growth factor receptor(ErbB)signaling pathway.Conclusion:Based on the FangNet platform,"symptom-drug"network was constructed for the collection of clinical cases of CHF treated by contemporary physicians,and the corresponding relationship between commonly used drugs of CHF and disease symptoms was explored.The mechanism of ameliorating chest oppression by salvia miltiorrhiza and semen lepidii might involve inhibiting cardiomyocyte apoptosis,inflammatory response and promoting neovascularization.
