Efficacy and Safety of Recombinant Human Brain Natriuretic Peptide in the Treatment of Type Ⅱ and Ⅳ Cardiorenal Syndrome:a Meta-analysis

Abstract:Objective:To evaluate the efficacy and safety of recombinant human brain natriuretic peptide(rhBNP)in the treatment of type Ⅱ and type Ⅳ cardio-renal syndrome(CRS).Methods:China National Knowledge Infrastructure(CNKI),WanFang,Wipro Chinese Full-text Database of Scientific and Technological Journals(VIP),PubMed,EMbase,Web of Science,the Cochrane Library were searched to collect randomized controlled trials(RCTs)of rhBNP in the treatment of type Ⅱ and type Ⅳ CRS.The search was from self-established database to december 16,2022.Meta-analysis was performed using RevMan 5.4 software.Results:A total of 18 articles were included,involving 1 400 patients.The results of meta-analysis showed that the clinical response rate of the experimental group was higher than that of the control group[RR=1.32,95%CI 1.24-1.40,P<0.000 01];NT-proBNP[MD=-852.88,95%CI-1 022.36--683.40,P<0.000 01],heart rate[MD=-11.11,95%CI-15.41--6.81,P<0.000 01],left ventricular ejection fraction(LVEF)[MD=5.03,95%CI 3.17-6.89,P<0.000 01],glomerular filtration rate(GFR)[MD=8.39,95%CI 6.99-9.78,P<0.000 01],serum creatinine(Scr)[MD=-34.36,95%CI-43.24--25.47,P<0.000 01],24 h urine volume[MD=346.70,95%CI 247.95-445.45,P<0.000 01],urea nitrogen(BUN)[MD=-4.74,95%CI-8.13--1.36,P =0.006],left ventricular end diastolic diameter(LVEDD)[MD=-2.18,95%CI-3.24--1.12,P<0.000 1]levels were significantly improved.The incidence of adverse reactions in experimental groups was increased,but the difference was not statistically significant[RR=2.43,95%CI 0.72-8.22,P =0.15].Conclusion:Existing evidence shows that rhBNP can significantly improve the cardiac and renal function of patients with type Ⅱ and type Ⅳ CRS,and alleviate the progression of the disease,but the attention should be paid to the adverse reactions.
