Correlation between Serum Uric Acid Level and Renal Function Injury and Renin-Angiotensin-Aldosterone System in Elderly Hy-pertensive Patients

Abstract:Objective To study the relationship between serum uric acid (UA)level and renal function and renin-angiotensin-aldo-sterone system (RAAS)in elderly patients with essential hypertension.Methods Four hundreds and Sixty-four elderly patients with hypertension in the department of Cardiology,Guang Anmen Hospital,China Academy of Chinese Medical Sciences from June 2015 to November 2013 were enrolled.There were 228 male and 236 female,aged (78.59± 7.96)years.Among them,one hundred and thir-ty-seven patients with hyperuricemia were as group A while 327 patients without hyperuricemia were as group B.The blood urea ni-trogen (BUN)and serum creatinine (SCr),angiotensin Ⅱ,aldosterone,angiotensin converting enzyme (ACE),plasma renin activity were observed.Results The levels of BUN,SCr,angiotensinⅡ and aldosterone in group A were higher than that in group B(P<0.05). There was no significant difference in the levels of Angiotensin converting enzyme and renin activity between two groups (P>0.05). Pearson correlation analysis showed that serum UA level was associated with BUN,SCr,angiotensin Ⅱ and aldosterone(P<0.01). Multivariate linear stepwise regression analysis showed that SCr,angiotensin Ⅱ were the main influencing factors of serum UA level . Conclusion Elevated UA levels in elderly patients with hypertension are closely related to renal function decline and activation of RAAS.Angiotensin Ⅱ is an independent factor of hyperuricemia.It may become a common target of antihypertensive and treatment of lowering UA by reducing angiotensin Ⅱ.
