The Value and Safety of Tirofiban in Elderly Patients with Diabetes Mel itus Complicated with Acute Myocardial Infarction undergoing Percutaneous Coronary Intervention

Abstract:Objective To study the clinical value and safety of tirofiban in elderly patients with diabetes mel itus(DM)complicated with acute myocardial infarction(AMI)undergoing percutaneous coronary intervention (PCI).Methods One hundred and twenty one elderly patients with DM complicated by AMI undergoing PCI from 2013 April to December 2014 in the Department of Cardiology in our hospital were randomly divided into two groups:The control group(n=60)treated with routine treatment,and the treatment group(n=61)treated with routine treatment plus tirofiban.The corrected thrombolysis in myocardial infarction (TIMI)frame count (CTFC),postoperative TIMI myocardial perfusion grade(TMPG),left ventricular ejection fraction(LVEF),Creatine Kinase MB(CK MB), postoperative platelet count and platelet aggregation rate,major adverse cardiac events (MACE)and bleeding complications were observed.Results After operation,CTFC was improved,which was better in treatment group than that in control group(P<0.05). The occurrence rate of TIMI grade 1,grade 2 and grade 3 flow were 8.3%,20.0%,71.7% in treatment group,which was better than that in control group(1.6%,8.2% and 90.2%,P<0.05).There was no significant difference in LVEF at 1 week after operation and CK MB at 6 hours,12 hours after operation between two groups (P>0.05).The platelet count and platelet aggregation rate in treatment group were higher than those in control group(P<0.05).The incidence of MACE at 30 days after operation in treatment group was lower than that in control group (P<0.05).The incidences of hemorrhage of digestive tract,skin and mucous membrane bleeding,in-tracranial hemorrhage,visceral bleeding bleeding complications in treatment group were lower than that in control group (P<0.05). Conclusion Tirofiban plus routine treatment can improve myocardial damage and myocardial reperfusion,prevent coronary thrombo-sis,reduce the incidence of adverse cardiovascular events after operation,and improve the quality of life and the prognosis in patients elderly patients with DM complicated by AMI undergoing PCI.
