Intravenous Thrombolytic Therapy with Recombinant Tissue Plasminogen Activator for Posterior Circulation Cerebral Infarction

Abstract:Objective To evaluate the effectiveness and safety of recombinant tissue plasminogen activator(rt PA)for posterior circu-lation cerebral infarction.Methods We respectively reviewed the patients with posterior circulation cerebral infarction in our hospital from January 2010 to December 2014.Fifty three subjects were enrol ed and twenty seven of them were treated with rt PA within 6 hours from the onset were as thrombolytic group and the other twenty six patients only accepted regular anti platelet aggregation or anticoagulant therapy without thrombolysis as non thrombolytic group.The neurological deficits,and the incidence of favorable outcome on the base line,24th hour,7th day and 12th week were compared.The incidence of hemorrhagic transformation (HT)and mortality within 12 weeks were analyzed.Results There was no significant difference in the National Institutes of Health Stroke Scale (NIHSS)score at the onset,24th hour,7th day,and 12th week between both groups.However,the incidence of early improve-ment in the thrombolytic group (25.9% at 24th hour and 48.1% at 7th day,respectively)significantly increased compared with the non thrombolytic group (0% at 24th hour and 0% at 7th day,respectively)(P=0.01).The incidence of HT at 24 th hour in the thrombolytic group (11.1%)had no significant difference with that in the non thrombolytic group (3.8%)(P=0.61),as also at 7th day (P=0.61).There were 5 (18. 5%)and 6 (23. 1%)death cases in the thrombolytic and non thrombolytic groups within 12 weeks from the onset,respectively,and no significant difference was observed (P=0. 68).Conclusion The rt PA intravenous thrombolysis increased the incidence of early neurological improvement in patients with posterior circulation cerebral infarction.in ad-dition,it increased the tendency of hemorrhagic transformation and reduced the risk of long term death in patients, compared with non thrombolysis group.
