Diagnostic Value of Transcranial Doppler for Intracranial Vascular Stenosis/Occlusion in Acute Cerebral Infarction

Abstract:Objective To investigate diagnostic value of transcranial Doppler compared with CT angiography in intracranial vascu-lar stenosis/occlusion in patients with acute cerebral infarction.Methods Forty patients with acute cerebral infarction were examined with transcranial Doppler and CT angiography to evaluate intracranial vascular stenosis/occlusion status.The CT angiography was used as standard,and diagnostic value of transcranial Doppler examination was analyzed.Results The internal carotid artery:Based on CT angiography examination,sensitivity,specificity,and accuracy of transcranial Doppler examination were 93.4%,99.2%,and 97.8%,respectively.The vertebral basilar artery:Sensitivity,specificity,and accuracy of transcranial Doppler examination were 82.6%, 99.4%,and 96%,respectively.The consistent rates of transcranial Doppler to CT angiography showed that bilateral middle cerebral arteries was 96.0%,anterior cerebral artery was 94.1% siphon carotid artery was 100.0%,terminate internal carotid artery was 94.4%, posterior cerebral artery was 81.8%,bilateral vertebral artery was 86.4%,and basilar artery 84.6%.Conclusion Transcranial Doppler examination was first choice for internal carotid artery.CT angiography examination was first choice for vertebral basilar ar-tery.The effective combination of two methods could significantly improve diagnostic accuracy rate of intracranial vascular stenosis/occlusion.
