Clinical Observation of Disposable Fistula Snares and in the Treatment of Adenogenic Ischiorectal Space Abscess

Abstract:Objective To investigate the clinical efficacy and safety of directional seton placement using a disposable fistula cutting and ligation device in the treatment of adenogenic ischiorectal space abscess.Methods A prospective randomized controlled study was conducted.Ninety patients with adenogenic ischiorectal space abscess admitted to The Third Affiliated Hospital of Liaoning University of Traditional Chinese Medicine(Liaoning Provincial Anorectal Hospital)between July 2024 and February 2025 were selected and randomly divided into two groups based on the odd or even number of their visit numbers,with 45 patients in each group.The treatment group underwent directional seton placement using the disposable fistula cutting and ligation device,while the control group received conventional incision and seton treatment.The clinical outcomes(total effective rate),false healing rate,operation time,postoperative wound healing time,and Wexner incontinence scores were observed and compared between the two groups.Results The total effective rate was 100%(45/45)in the treatment group and 91.1%(41/45)in the control group,with no statistically significant difference(P>0.05).The false healing rate in the treatment group(4.4%)was significantly lower than that in the control group(22.2%),and the difference was statistically significant(P<0.05).The operation time in the treatment group was significantly shorter than that in the control group(P<0.05).The postoperative wound healing time in the treatment group was shorter than that in the control group,but the difference was not statistically significant(P>0.05).The postoperative Wexner incontinence score in the treatment group was significantly lower than that in the control group(P<0.05).Conclusion Directional seton placement using the disposable fistula cutting and ligation device for adenogenic ischiorectal space abscess demonstrates definite clinical efficacy while effectively preserving anal function.This method offers advantages such as convenience,minimal invasiveness,and a low false healing rate.By reducing tissue damage through minimized trauma,and lowering the false healing rate via precise lesion management and adequate drainage,it fundamentally shortens recovery time and reduces hospital stay.Furthermore,it enhances treatment efficiency by simplifying procedural steps,saving operation time for both surgeons and patients,and reducing surgical difficulty,demonstrating comprehensive advantages in clinical practice.
