Effects of Platelet-Rich Plasma Injection into the Dorsal Root Ganglion on Cytokine Profiles During Herpetic Neuralgia

Abstract:Objective:Exploring the effects of ultrasound combined with C-arm X-ray on pulsed radiofrequency of dorsal root ganglia and injection of platelet-rich plasma on cytokines during the subacute stage of herpetic neuralgia.Method:A total of 49 patients with herpetic neuralgia who were admitted to the Dongguan First Hospital Affiliated to Guangdong Medical University from July 1,2023 to December 31,2024 were selected as the research subjects.They were randomly divided into the observation group(25 cases)and the control group(24 cases)using the random number table method.Both groups underwent ultrasound-guided combined C-arm X-ray guided pulsed radiofrequency pain relief surgery under local infiltration anesthesia.At the end of the radiofrequency procedure,the observation group(PP group)was given 1%lidocaine 2 mL+platelet-rich plasma 2 mL,totaling 4 mL,while the control group(P group)was given 1%lidocaine 2 mL+0.9%sodium chloride 2 mL,totaling 4 mL.The levels of cytokines[(pro-inflammatory factors:interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor(TNF),anti-inflammatory factor:interleukin-10(IL-10),oxidative factors:indole 2-hydroxylase(IDO),total plasma homocysteine(tHcy),melatonin(MLT)]before the operation and 7 days after the operation were recorded.Before and 1,7 days,1,3,and 6 months after the operation,the nature of pain(pain numerical rating scale(NRS)and attack frequency),and secondary injuries(hematoma,nerve,pneumothorax,infection).Result:Compared with the preoperative levels,the levels of factors IL-6,IL-8,TNF,IL-10 and melatonin(MLT)in both groups decreased significantly at 7 days postoperatively,while the levels of indoleamine 2,3-dioxygenase(IDO)and plasma total homocysteine(tHcy)increased significantly,with statistically significant differences(P<0.05).Compared with the P group,the levels of factors IL-6,IL-8,TNF,IL-10 and melatonin(MLT)in the PP group decreased more significantly at 7 days postoperatively,while the levels of indoleamine 2,3-dioxygenase(IDO)and plasma total homocysteine(tHcy)increased more significantly,with statistically significant differences(P<0.05).Compared with the P group,the VAS scores and attack frequencies in the PP group at 1,3 and 6 months postoperatively were lower,with statistically significant differences(P<0.05).No adverse events were observed in either group.Conclusion:The combination of pulsed radiofrequency of dorsal root ganglion of spinal nerve and platelet-rich plasma in the treatment of acute herpes zoster neuralgia has a good effect,reduces inflammatory factors during the acute herpes zoster neuralgia period,and has a certain intervention effect on the transformation to PHN.
