Application of Dexmedetomidine and Ropivacaine in Patients Undergoing Radical Resection of Colorectal Cancer with Subcostal Transversus Abdominis Plane Block

Abstract:Objective:To explore the effect of dexmedetomidine(Dex)combined with ropivacaine in patients undergoing subcostal transversus abdominis plane block(TAPB)for radical resection of colorectal cancer(CRC).Method:A total of 100 patients with subcostal TAPB who underwent radical CRC resection and were admitted to Gutian County Hospital from October 2022 to September 2024 were selected as the research subjects.They were divided into the study group(n=50)and the control group(n=50)according to the random number table method.The control group was treated with ropivacaine,and the study group was treated with Dex combined with ropivacaine.The dosage of anesthetic drugs,hemodynamic indicators,sedative effects,pain degrees and adverse reactions were compared between the two groups.Result:The dosage of remifentanil,the dosage of propofol,and the number of compressions of patient-controlled intravenous analgesia(PC I A)in the study group were less than those in the control group,and the first compression time of PCIA was longer.The differences were statistically significant(P<0.05).Before plane block(T1),at the beginning of the surgery(T2),and 30 minutes after the start of the surgery(T3),the heart rate(HR)and mean arterial pressure(MAP)in both groups showed a trend of first decreasing and then increasing,and the study group was higher than the control group,with statistically significant differences(P<0.05).The Ramsay sedation score(RSS)of the study group 2 hours after the operation was lower than that of the control group,and the difference was statistically significant(P<0.05).The RSS scores of the study group at 6 hours and 1~2 hours after the operation were higher than those of the control group,and the difference was statistically significant(P<0.05).At 2 hours,6 hours and 12 hours after the operation,the Numerical Rating Scale(NRS)scores in the resting and active states of the two groups showed an upward and then downward trend,and the score of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:For patients with subcostal TAPB after radical CRC surgery,the use of Dex combined with ropivacaine can reduce the dosage of anesthetic drugs,improve the sedative effect,alleviate postoperative pain,promote hemodynamic stability,and has relatively high safety.
