Study on the Efficacy of Nasal High-Flow Humidified Oxygen Therapy in Patients with COPD Exacerbation Complicated by Type Ⅱ Respiratory Failure

Abstract:Objective:To assess the efficacy and safety of transnasal high-flow humidified oxygen therapy(HFNC)in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)combined with type Ⅱ respiratory failure(Ⅱ-RF).Method:A randomised,single-blind,single-centre controlled trial design was employed.120 patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)classified as Ⅱ-RF,admitted to Macheng People's Hospital between January 2024 and January 2025,were enrolled.They were randomly assigned to either the high-flow nasal cannula(HFNC)group(n=60)or the non-invasive positive pressure ventilation(NIPPV)group(n=60).Primary outcomes included clinical efficacy,blood gas analysis parameters,vital signs,and adverse reaction incidence rates.Result:The overall response rate in the HFNC group was significantly higher than that in the NIPPV group,with a statistically significant difference(P<0.005).Post-treatment,the HFNC group demonstrated superior improvements in PaCO2 reduction,PaO2 elevation,and pH correction compared to the NIPPV group.Respiratory rate and heart rate improvements were also markedly greater in the HFNC group,with statistically significant differences(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:HFNC can effectively improve the blood gas index and vital signs of patients with AECOPD Ⅱ-RF,improve the clinical efficacy,and has good safety,which has clinical application value.
