Clinical Analysis of Laparoscopic and Choledochoscopic Combined Treatment for Intraoperative Primary Suturing in Cases of Gallbladder and Common Bile Duct Stones

Abstract:Objective:To investigate the clinical outcome of one-stage suturing of the common bile duct during laparoscopic combined choledochoscopic cholecystectomy and common bile duct exploration.Method:A total of 80 patients undergoing cholecystectomy combined with common bile duct stone removal surgery at Xinyi Traditional Chinese Medicine Hospital between September 2022 and December 2024 were enrolled.Patients were grouped according to the common bile duct surgical approach selected.Patients undergoing primary closure 40 cases were assigned to the Primary Closure Group,while those receiving T-tube drainage 40 case constituted the T-tube Drainage Group.Surgical characteristics,pain management outcomes,and postoperative complication rates were analysed between the two groups.Result:Compared with the T-tube drainage group,surgical procedures,postoperative bowel function recovery,hospital stay duration,hospital costs,complication incidence,Visual Analogue Scale(VAS)scores at various postoperative time points,and postoperative analgesic usage rates were lower in the primary suturing group,with statistically significant differences(P<0.05).There was no statistically significant difference in intraoperative blood loss between the two groups(P>0.05).Conclusion:In the process of cholecystectomy and choledochoscopic exploration and stone extraction,the use of choledochal one-stage suture not only helps to promote the rapid recovery of the patient,but also effectively reduces the medical expenses during hospitalisation.At the same time,it also shows positive clinical value in relieving postoperative pain symptoms and reducing the risk of various complications.
