Comparison of the Clinical Outcomes of Minimally Invasive Percutaneous Plate Osteosynthesis at Different Timing for Tibial Plateau Fractures

Abstract:Objective:To investigate the clinical efficacy of minimally invasive percutaneous plate osteosynthesis(MIPO)performed at different time points for the treatment of tibial plateau fractures.Method:A total of 86 patients with tibial plateau fractures admitted to Huai'an First People's Hospital from January 2022 to January 2024 were enrolled.Patients were randomly assigned to either the early group(surgery within≤3 days after injury,n=41)or the subacute group(surgery at 4~7 days after injury,n=45).All patients underwent standardized MIPO procedures and were followed up for 12 months postoperatively.The knee joint function scores[HSS knee joint score,Lysholm knee joint score scale,International Knee Documentation Committee knee joint assessment scale(IKDC)],the incidence of complications,the quality of imaging repositioning,the length of hospital stay,the operation time,and the inflammatory indicators[C-reactive protein(CRP),interleukin 6(IL-6)]of the two groups were compared.Result:At 12 months postoperatively,the early group had significantly higher HSS,Lysholm,and IKDC scores compared to the subacute group.The incidence of overall complications,as well as postoperative IL-6 and CRP levels,were significantly lower in the early group.The early group also showed a higher rate of excellent radiographic reduction and shorter hospital stays than the subacute group,with statistically significant differences(P<0.05).Conclusion:Early MIPO surgery after injury can significantly improve the knee joint function of patients with tibial plateau fractures,reduce inflammatory responses and the risk of complications,shorten the hospital stay,and provide evidence-based basis for optimizing the selection of surgical timing.
