Effect of Pelvic Floor Reconstruction Combined with Autologous Anterior Vaginal Mucosal Flap Urethral Suspension for the Treatment of Female Patients with Stress Urinary Incontinence

Abstract：Objective:To investigate the clinical efficacy of pelvic floor reconstruction combined with autologous anterior vaginal mucosal flap urethral suspension in the treatment of female stress urinary incontinence(SUI)and its impact on pelvic floor function and urodynamic levels.Method:A total of 100 patients with grade Ⅲ SUI admitted to Fuqing Oufei Hospital from January 2022 to March 2024 were prospectively selected and randomly divided into an observation group(n=50,pelvic floor reconstruction surgery combined with autologous anterior vaginal mucosal flap urethral suspension surgery)and a control group[n=50,pelvic floor reconstruction surgery combined with transforaminal tension-free mid segment suspension surgery(TVT-O)]using a random number table method.The efficacy,impact of pelvic floor dysfunction questionnaire(PFIQ-7),pelvic floor dysfunction questionnaire(PFDI-20)scores,bladder compliance(BC),maximum urine flow rate(Qmax),and incidence of complications were compared between two groups.Result:There was no significant difference in total efficacy after treatment between the two groups(P>0.05).One year after surgery,both groups showed a decrease in PFIQ-7 and PFDI-20 scores,while BC and Qmax increased,with statistically significant differences(P<0.05).There were no statistically significant differences in PFIQ-7,PFDI-20 scores,BC,and Qmax between the two groups(P>0.05).The incidence of complications in the observation group was lower than that in the control group,with a statistically significant difference(P<0.05).Conclusion:Pelvic floor reconstruction combined with autologous anterior vaginal mucosal flap urethral suspension and pelvic floor reconstruction combined with TVT-O have good therapeutic effects on female stress urinary incontinence,and can improve patients'pelvic floor function and urodynamic level.Pelvic floor reconstruction combined with autologous anterior vaginal mucosal flap urethral suspension has a lower incidence of complications and better safety.
