The Effect of Ultrasound-Guided Lumbar Sacral Plexus Block Combined with General Anesthesia in Elderly Patients Undergoing Hip Replacement Surgery

Abstract:Objective:To explore the effect of ultrasound-guided lumbar sacral plexus block combined with general anesthesia in elderly hip replacement surgery.Method:According to the random number table method,103 elderly patients who underwent hip replacement surgery at Changshu Second People's Hospital from October 2022 to October 2024 were selected and divided into a control group(n=51)and a combination group(n=52).The control group received general anesthesia,while the combination group received lumbar sacral plexus block under ultrasound guidance in addition to general anesthesia.The anesthesia effects,hemodynamic indicators,visual analog scale(VAS),and postoperative delirium incidence were compared between two groups.Result:The excellent rate of anesthesia effect in the combined group was higher than that in the control group.The incidence of delirium was lower than that of the control group,and the difference was statistically significant(P<0.05).At the T0 time point,in the levels of hemodynamic indicators MAP and HR between the two groups(P>0.05).At time points T1 T0 T3,the levels of MAP and HR in both groups showed a downward trend compared with T0.However,compared with the control group at time points T1 to T3,the levels of MAP and HR in the combined group were higher during the same period.Moreover,at 2 hours,6 hours,12 hours and 24 hours after the operation,the VAS scores of the combined group were all lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of ultrasound-guided lumbar sacral plexus block combined with general anesthesia in elderly hip replacement surgery can improve anesthesia effectiveness,promote hemodynamic stability,and reduce postoperative pain.It also has good safety and can reduce the incidence of delirium.
