Analysis of Risk Factors for Nosocomial Infection in Patients with Cerebral Infarction Complicated with Coronary Heart Disease after Percutaneous Coronary Intervention

Abstract:Objective:Based on clinical practice experience,this paper explores the risk factors of nosocomial infection in patients with cerebral infarction complicated with coronary heart disease after percutaneous coronary intervention.Method:A total of 268 patients with cerebral infarction complicated with coronary heart disease who underwent percutaneous coronary intervention in the Second Affiliated Hospital of University of South China from November 2023 to February 2025 were selected.They were divided into the infection group and the non-infection group according to whether nosocomial infection occurred after the operation.The incidence of nosocomial infection after PCI in patients with cerebral infarction complicated with coronary heart disease was statistically analyzed.The univariate factors of nosocomial infection after PCI in patients with cerebral infarction complicated with coronary heart disease were analyzed,and the multivariate logistic analysis was conducted on the factors influencing nosocomial infection after PCI in patients with cerebral infarction complicated with coronary heart disease.Result:Among 268 patients with cerebral infarction complicated with coronary heart disease,32 cases had nosocomial infections,with an incidence rate of 11.94%.After univariate analysis,there were statistically significant differences in factors such as age,cardiac function classification,invasive procedures,the number of stent implants,mechanical ventilation,and indwelling urinary catheters between the infected group and the non-infected group(P<0.05).The remaining factors such as gender,body mass index(BMI),smoking history,drinking history,combined hypertension,combined diabetes,combined hyperlipidemia,and puncture site showed no statistically significant differences between the two groups(P>0.05).The factors with significant differences among the single factors influencing nosocomial infections in patients with cerebral infarction complicated with coronary heart disease after PCI were respectively taken as independent variables and dependent variables for quantitative assignment.After logistic regression analysis,factors such as age≥70 years old,cardiac function grade Ⅲ~Ⅳ,invasive procedures,the number of stent implants≥2,mechanical ventilation and indwelling urinary catheters were all independent risk factors affecting nosocomial infections in patients with cerebral infarction complicated with coronary heart disease after PCI,and the differences were statistically significant(P<0.05).Conclusion:The incidence of nosocomial infection after percutaneous coronary intervention in patients with cerebral infarction complicated with coronary heart disease is relatively high.The risk factors are mainly related to age≥70 years old,cardiac function grade Ⅲ~Ⅳ,invasive procedures,the number of stent implants≥2,mechanical ventilation and indwelling urinary catheters,etc.Sufficient attention should be paid to the above factors in clinical practice to reduce the incidence of nosocomial infection after PCI in patients with cerebral infarction complicated with coronary heart disease.
