Efficacy and Safety of Intersphincteric Approach Drainage and Multi-Incision Drainage in the Treatment of Perianal Abscess

Abstract:Objective:To explore the efficacy and safety of intersphincter approach drainage and multi-incision drainage in the treatment of perianal abscess(PA).Method:The clinical data of 113 PA patients admitted to Xi'an Gem Flower Changqing Hospital from May 2021 to June 2023 were selected.They were divided into two groups according to the treatment plan.The control group(56 cases)underwent multi-incision drainage,while the observation group(57 cases)underwent intersphincter approach drainage.The clinical efficacy,pain degree,exudate status,exudate growth factors[basic fibroblast growth factor(bFGF),epidermal growth factor(EGF),transforming growth factor-β1(TGF-β1)],perioperative recovery,recurrence and formation of anal fistula were compared between the two groups.Result:The total effective rate of the observation group was higher than those of the control group,and the difference was statistically significant(P<0.05).Compared with the control group,the wound pain degree and exudate score of the observation group decreased at 1 d,7 d,14 d and 21 d after operation,and the difference was statistically significant(P<0.05).The expression levels of EGF,TGF-β1 and bFGF in the wound exudates of the observation group were higher than those of the control group at 7 d,14 d and 21 d after operation,and the differences were statistically significant(P<0.05).Compared with the control group,the perioperative recovery index time of the observation group was shorter,and the difference was statistically significant(P<0.05).Compared with the control group,the postoperative recurrence rate and anal fistula formation rate in the observation group decreased,and the difference was statistically significant(P<0.05).Conclusion:Intersphincter approach drainage has a significant effect in the treatment of PA.It can shorten the recovery process,relieve symptoms,and reduce the risk of recurrence and anal fistula formation.
