Effect of Dapagliflozin in Patients with Type 2 Diabetes Mellitus Who Are Not Well Controlled by Insulin Treatment

Abstract:Objective:To explore the application effect of dapagliflozin in patients with type 2 diabetes mellitus(T2DM)who have an unsatisfactory response to insulin treatment.Method:80 patients with type 2 diabetes mellitus(T2DM)who had a poor response to insulin treatment and were admitted to Dongtai Hospital of Traditional Chinese Medicine from January 2022 to June 2024 were selected as the research subjects.They were divided into the study group and the control group according to the random number table method,with 40 cases in each group.The control group was given liraglutide injection in addition to the conventional treatment,while the study group was given dapagliflozin Tablets in addition to the conventional treatment.The application effects were compared between the two groups.Result:After treatment,the fasting plasma glucose(FPG),2-hour postprandial blood glucose(2 hPG),and glycated hemoglobin ratio(HbA1c)in both groups decreased,and the study group were lower than the control group,with statistically significant differences(P<0.05).After treatment,the fasting C-peptide value(FCP),2-hour postprandial C-peptide value(2 hCP),fasting insulin value(FINS),and 2-hour postprandial insulin value(2 hINS)in both groups increased,and the study group were higher than those in the control group,with statistically significant differences(P<0.05).After treatment,the insulin β-cell function index(HOMA-β)in both groups decreased,and that in the study group was lower than that in the control group,with statistically significant differences(P<0.05).After treatment,the insulin resistance index(HOMA-IR)in both groups increased,and it was higher in the study group than in the control group,with statistically significant differences(P<0.05).Conclusion:The incorporation of dapagliflozin into the foundational the rapeutic regimen for T2DM indiciduals who exhibit an unsatisfactory response to insulin constittes a therapeutic paradigm with pronounced advantages.By leveraging a glucose-lowering mechanism that circumvents insulin depedence,dapagliflozin enables rapid intensification of glycemic regulation while concurrently fostering recuperation of islet functional capacity,thereby delivering multidimensional clinical benefits to this patient population.
